Creating Forms in outlook - "From"

When I crreated my form in Oulook, I included an address in the "From" field,
but when I publish the form and go to use it the "From" field is not
populated. How do I get the "From" field to stay in my form?

What's the point? Unless you're using Exchange, the From field is
meaningless. It does not reflect the sending address.
 
Because I have several mailboxes for different projects I work on and when I
send the messages I want the "From" to indicate the name of the mailbox, e.g.
(e-mail address removed) and not my name or main mail box. I believe I am using an
exchange.
 
One of the points in creating the form was so that I didn't have to always
add in the "from" address when I was sending the messages. I want to create a
form for each mail box with one less step involved.

But that's not what the "From" field is for. The "From" field is for when
you are sending on behalf of another user....
 
Thank you for your response. But I am creating a form to be used over and
over again and it allows me to enter the "From" when I create the form, but
when I open it to use it again, the "From" is not populated. There should be
a way to create the form where the "From" field remains populated. It is
probably a simple fix, I just need to find out what that is. If I do, I'll
post it here.
