Creating Form to set values to use in Macros

I have a form with many fields that represent decimal multipliers, to use in determining discounts on other fields. I only need one database and one record to hold these values, but don't want more. Access automatically generates more fields. How can I make them fixed, one input in the serveral fields for and only one record. Similar to using $A$1 for referancing the same cell for many cells.
 
It's kind of hard to get a handle on what you already have and what you are
trying to do, based on your description. Given that a spreadsheet already
provides the functionality you are seeking, why not use a spreadsheet? Or,
put another way, what is it that you expect Access to do?
