Creating Check Boxes

I'm creating a Powerpoint presentation and they would like to have check boxes where the person filling in the information can click within the check box. Is this possible?
 
Go to View | Toolbars | Controls

Select the check box from the toolbar, then place it on
the slide.

You can add checkboxes using the controls toolbox as has been suggested, but
there's got to be more to the story, no?

User checks a checkbox ... then what?
Does the choice need to be saved to file in some way or does it determine the
flow of the presentation or ...????
