Creating a Survey

I need to (a) create a database based on survey results
and (b) create charts based on the data entered.

Here was my plan of action:

(the survey has a question then check boxes for agree to
disagree)

1. make a label box for the question
2. create a check box for every reply
3. each reply will be a true/false
4. calculate the true/false answers
5. create a chart based on these results.

Do I have to have each check box as a reply and how do I
make charts?
