Creating a questionnaire...

I have a questionnaire for patients to fill out that I would like to send via
email. How do I format the questionnaire so the patients can respond on the
same form? For example, a "yes" or "no" question, I want the patient to be
able to chech a box, or for a question, I want a text box that a patient can
type a response into. I'm completely new to computers, if anyone could help,
that would be great.
 
What you are talking about is what Word calls an "online form." Check this
