creating a pop up box to run a form control

I have a form where certain fields disappear or appear depending on whether
you check the March2006 checkbox or the July1999 checkbox. Currently the
checkboxes are just form-based and are not in the table, but I can change
that if necessary. This is because there are two different reports being
created off this one form and they differ slightly depending on the version
of the form.

I would like a message to pop up when you open the form to ask which version
of the form you would like to use (or if you would like to use x version of
the form and if you choose no, the other version is automatically selcted).

How would I go about doing this?
 
I am confused by "version of the form" ... data is always
STORED in tables -- forms just provide a convenient way to
look at and edit that data.
