Creating a new Form from an older form formatt

Problem: Trying to make a 06-07 update form for my members, and I want to use
the same formatt and wording like last year; but I want to get my new
information from a differnt table. How do I create a form with the same
template/wording but import from another table?

Please help
 
Open the form in design view, click on menu VIEW - Properties. Edit the
Record Source.

Better yet create query and use it for the record source.
