Creating a fax template W97, how to create entry fields?

Or, if you just tell me what I should be using as search terms in help,
that would at least be a great start-

I want to simplify my fax cover sheet- right now, I type in names, etc and
it moves the lines based on how many characters I type.

I want to make two changes:

(1) I want to add some fields, where they are fixed size (empty) and I can
click on them and add the text I want (names, etc) without moving anything
else on the page, and

(2) a larger, multi-line text box, that will always have a minimum size
(say, 12 lines) but will also autoexpand down if I add more text than that.

I looked at bookmarks and fields, but don't see anything that would be
blank and type-able. I also looked at the VBA toolbar text field, but it
looks more like a "picture" sitting on top of my document, rather than
having one professional looking template.

Can anyone point me in the right direction?

Have you tried using a TABLE, with the row height set to "exactly" in the
first case, and "at least" in the second?

A FRAME from the Forms toolbar would also be a possibility.
