Creating a contents / index page for a report

I have an addressbook report which when printed is over
200 pages long. The report is grouped by two levels, and
is further sorted on one of the fields (company name).

I want to create an index page or a contents page that
will allow me to show the page number that an individual
company name record starts on.

For example
The record for "Joe's Architects" is on page 3. I would
like the index to show this.


Any help would be much appreciated.
