Create Survey that will send data back to me.. Best method?

I need to create a survey that my customers can input and automatically have
the data sent back to me.
 
Create an HTML form with a text box, drop-down list, check box, or set of
radio buttons for each question. Then, right-click the form area, choose Form
Properties, and specify where you want the results to go (a file, mail, or a
database).
