Create QA Form

Hi,
I am trying to create a form to assist in the evaluation of work done by
employees. The QA form has 4 sections with 3 - 6 questions in each section.
The questions do not have the same weight (some are worth 3 points, some 4pts
etc). Each employee will be evaluated 5 times/month. Can someone provide some
direction on how to create this in Access?
 
Reporting on weighting would not be in the data collection form but in your
report following data collection.

A Quality Assurance form like this is something new to me. I would like to
know more.
