Create PDF form and link answers to an Access Database

I'm trying to create a survey form. I will use Adobe Acrobate to convert it
to a pdf file, which is easier to send, but I want the fields used for the
answers to correlate to my database, so that I don't have to re-enter the
information.

This is really an Acrobat question rather than an Access one.

If you know how to create a PDF document that contains a form that will
be "live" when users open it so they can type in the answers, you're
well ahead of me, and the Acrobat documentation will tell you how to get
the answers out of the completed forms into a database or a file that
Access can import.
