Create Folder not working

Trying to create a new folder.
When I open say My Documents and right click in an open area I do not see
the Create Folder option in the box that pops up.
Ok, I then try to click on the navigation bar at left to create new folder
and it doesn't work. It just blunks at me letting me know I've done
something wrong.
There's a small red "star" showing up next to create new folder on the left
nav bar.
Any suggestions?
 
Does the window have a menu bar? What is displayed on the File drop down
menu?

When I right click I get a box opens with:
arrange icons by
refresh
paste - grey out
pasteshortcut - grey out
undo move
restore files in folder - part of moxy pro
properties

Clearly there is an item missing from the Menu as you well know. I have
tried to find out where these menus are stored with no success. Sorry I
can not help.
