create a question that can be answered in the same text in outlook

I'm looking to email a questioneer out that can be replied to in the same
text (email body).
Rate my service from 1 to 5, 5 being best.

Was my response time consistent with your expectations? ____
How ould you relate my proffessional advice? ____

And what version of Office are you using? Access 2007 + Outlook 2007 can do this nicely, but not earlier versions.

Then you probably ought to consider using a web-based survey tool if you want to be able to easily tabulate the results.
