Create a queries with many yes/no check boxes

I'm trying to create a query that shows answers from an observation form.

the form looks like the following:

1. Employee wearing a hard hat ? Yes No N/A Score 1 Score 3
Score 10

Each one is a check box field. So 6 fields X 25 questions = 150 check boxes

There are approx. 25 questions in a form. I have a total of 145 questions
in all the forms. I'm trying to get the answers from the first form.

How do I get the answers, if the box is checked for all questions?

website.

This design is going to be a nightmare to summarize. But if you
really want to, have at it.
