Cover page fields

After creating a custom cover page in "My Faxes, Common
Cover Pages", then trying to send a fax from "My Faxes,
Send cover page fax" the field options do not include all
that were selected from the available options. If I fax
from word I don't have a phone book.
 
I'm using the fax software from wondows 2000 professional,
accessories, communications, fax and not using outlook, but
it's office XP professional

If you did not install Outlook, then the Fax Service will be using your
Windows Address Book. So are you saying that when you use the "Send Cover
Page Fax" Wizard, you have an address book, but you don't if you send from
Word?
Make sure you are configured correctly:
