Corrupt Select Names Dialog

We are using Outlook 2002 (10.4219.4219) SP-2 on NT4SP6A with an Exchange
server.

When a user has started a new email message, activates her address book, and
selects her contacts folder, NOTHING appears in the select names dialog. If
you scroll up or down, or even click into the field, there's NOTHING there.
But if you start typing an addressee's name, the list will populate.

This started happening after this user had just synched her Blackberry.

We have already tried un/reinstalling Outlook, changing various settings in
Outlook, and recreated her Mail profile.
 
Did you add the Outlook Address Book Service to this profile and configure
it correctly?
 
Yes. Everything was working fine until after she synched, and ended up with
the additional entries.

Remove the Outlook Address Book.
Restart Outlook.
Re-add it and configure it correctly to display the Contact Folder(s) you
want to see.
