Hi

Is the BCM add-on enabled? If it is you should see a Business Tools Menu.
If it is not present, go to Help > About Microsoft Office Outlook and check
for BCM under disabled items

That, or your Outlook form cache is corrupt. Try deleting it and BCM
will recreate the forms.
