CommandBar on Custom Form Disappears

I had used a custom Outlook form for a while until recently when the
CommandBar disappeared.

When I try to save data to Outlook from Access I get the following error
message:

The Custom form could not be opened. Outlook will use an Outlook form
instead. The form required to view the message cannot be displayed. Contact
your administrator.

When I try to open a form from Outlook, I get the following error message:

The form you selected could not be displayed. The form required to view this
message cannot be displayed. Contact your administrator.

I have redesigned and republish the forms and the error persists.

I will appreciate help in solving this problem.

That warning suggests that the forms cache is corrupted. Have you cleared it?

Thanks for you response.
I tried clearing the cache following the steps below outlined in Outlook
2000 Help but did not see the Custom Forms button (Step 3).

Is there another approach to clearing the cache or is there a way to make
the Custom Forms button display?

Remove a form from a forms library
1. On the Tools menu, click Options.
2. Click the Other tab, and then click Advanced Options.
3. Click Custom Forms.
4. Click Manage Forms.
5. Select the form in either your personal or organizational forms library,
and then click Delete.
