Command Toolbar No Longer Works

When pressing Repy, Forward, Save, Save and Close and
various other buttons on the standard toolbar, the command
fails to execute. Closing the item, and then choosing
Reply from the Right Click context menu or from the menu
bar does work. How do I get the commands on the toolbar
to work again?
 
Close Outlook. Find and rename outcmd.dat to .old. (If using Windows 2000
or XP, you will need to enable searching hidden and system folders.)

Once you have done so, reopen Outlook and it should open fine. You will
need to rebuild any custom toolbars or customizations as the file outcmd.dat
controls how the toolbars display. When it becomes corrupted, it will cause
Outlook to hang because it cannot create the toolbars that you expect.

I closed Outlook and renamed all instances of outcmd.dat
to .old. The problem still exists. Now is seems worse.
It used to be that to workaround the failing toolbar, I
had to Cancel the message, save it to Drafts, then send it
from Drafts. Now this doesn't work either. Almost virus
like, deterioration. Running NAV 2003 w/ latest defs,
though.

I also tried renaming the formscache file and this didn't
help either. Although I have no Custom forms, these are
the default MS OL forms.
