Found this in the Knowledge base:FIX: A USB Device Appears
with a Yellow Exclamation Mark (Code 31) in Windows Device
Manager

It says that a hotfix is available, how/where do I find
this, thought maybe it would be a downloadable file.

Hi,

I have the same problem as you. I have a mobo with USB 1.1
host, and a PCI card with USB 2.0 host. The problem is
that in device manager *all* USB host controllers items
are with a yellow circle and exclamation point and I
cannot use *any* USB device.

Please, is urgent for me, I have no scanner, no CD writer
and no colour printer... anything.

I have installed SP4 and would like to download the
hotfix, but where?

Any advice is highly appreciated. Thanks in advance.
 
Try disable onboard USB.
boot PC hit "del" or "F1" Key to get into BIOS settings - Follow your PC or Motherboard manual.
and find I/O settings.
If you still have confilict on USB Host Controler, change your USB Host Controler's
resources settings.

All the things you have tell me have been tested with no
sucess. But thanks anyway.

I have downloaded the hotfix by calling Microsot, and the
problems has gone... BUT I am still experiencing problems
with USB devices, because they are not recognized by
W2000.

I have one more clue on this; the following registry key
is empty:

/HKLM/SYSTEM/CurrentControlSet/Enum/USB

So, there is no USB host controller!! They all had gone!!
In fact, on this key there should be one USB 1.1 host
controller (Intel) for motherboard and another one (VIA)
for a PCI to USB 2.0 card.

I think the problem is here, Windows 2000 has deleted
these keys, so it is impossible to USB work properly. The
big question is, WHY?

In brief: I have installed Service Pack 4 and the hotfix
to correct this (KB 822368) but I still missed USB.

Any idea on how to tell Windows 2000 server to rescan the
USB host controller?

how about use your USB2.0 PCI card's Installation program to reinstall driver.
(from CD or download from manufacture web site)

The driver is present and loaded, but still no response if
I connect any USB device in any port (neither on board USB
host nor PCI card).

Simply, the USB host controller is not present and the
registry key which manages USB is e-m-p-t-y. Is not about
drivers, is a bit more serious. I have lost an entry in
the registry and I do not know how this happened. It is
impossible for Windows to detect any USB device if there
is nothing in the registry to tell Windows that there is a
USB host controller.

Any other idea to get back the lost keys? Why Windows does
not detect the USB host controller?

do you mean you did uninstall both of usb host controller drive,
reboot and then reinstall usb2.0 PCI card adapter driver?

by the way what is your motherboard or pc manufacture and model number,
and what is your USB2.0 PCI card adapter's manufacture and model?

No, I didn't unistall anything. USB devices stopped for
working (I was using the onboard USB host 1.1 rev) so as
I needed to use some USB devices (scanner, printer and cd
writer) I bought a new PCI card USB 2.0, that was
correctly detected by Windows 2000 but does not work,
also. It is like all my USB ports are death, there is no
response from them.

On the other hand, drivers are detected and loaded O.K.
and an IRQ is assigned to them.

Motherboard: ASUS P2B-S
USB card: VIA chipset

Hello,

John, I suggest you to request from Microsoft the hotfix,
because the version I have is for spanish installed O.S.
and this hotfix changes some files that are relevant for
the correct O.S. operating, and it is not recommended to
mix between differente language releases.

Microsoft did not ask me anything, just simply an e-mail
address where sending the hotfix. I told them about the
problem I had and I mentioned the KB 822368 and that's
all.

Anyway, if you are interested in this hotfix, tell me and
I will forwarwd to you.

I am looking for this also. I was hoping someone had
posted a reply. I don't see why we need to pay support
fees for this. This is clearly a windows defect and a
downloadable file should be posted with Article 822368. if
you find out where to find this file please pass on the
info.
