Checkmarks create with Form Toolbar disappear

Created a form in Word 2000 which used the checkmark boxes in Forms Toolbar. When done, I protected the "form" saved and exited. Open the form and checkmarked the boxes to see if it worked and it did. Exited and didn't save. Sent the form to someone in the office as an attachment in Office and when they opened the form and tried to checkmark the boxes, the boxes actually disappeared from the screen (even though the form was still protected).

Has anyone ever seen this happen? Also, found out that a form I had done previously many months ago, that I placed in our document management software has the same problem. You open it, and the checkmark boxes disappear.

Anyone have any ideas???? I didn't save the document as a .dot, but just as a regular .doc. That doesn't make any difference does it?

I do remember seeing this come up before, and I was even able to
duplicate it at the time. But I can't remember what the particular factor
was that caused the problem...

Have you inserted these checkboxes into anything, such as a textbox,
table cell, frame, etc.?

You probably do want to save it as a template, but that doesn't effect what
is happening here. If the checkboxes disappear when someone clicks on them
and types an X, then the document is _not_ protected. If it is protected,
then they aren't disappearing. Did the user try to print out? Which version
of Word is the user using?

What is your document management software? Does it change the structure of
the file?

What happens if you use a password to protect your document for forms?
(Tools => Protect Document instead of clicking on the lock button)
