Checkboxes

I want to add a series of checkboxes to an e-mail that someone can hit reply, check the boxes, and send it back. One problem is with security settings using the forms or control toolbox toolbars doesn't necessarily work. Other ideas??
 
It's possible to make this work within your organization if you use
Microsoft Exchange as your mail server, using custom Outlook forms. So what
is your mail enviornment?

Correct. You can't really use mail messages reliably in this fashion to
collect information unless you're in a homogeneous Exchange environment.
Consider using a Word attachment protected for form field entry or a web
page instead.

sending this out to other servers, some not necessarily using M.E. Even if
I set the e-mail up correctly and then send it to myself, the security
settings or format does not allow the boxes to display.
