Checkbox Groups in Outlook Form

I have created an Outlook custom form and have created some checkboxes. I want people to pick one or the other options by checking the appropriate checkbox but not both. How do I create a checkbox group to do this? Right now the form will let me check both boxes and when I email the form to myself the checkbox is unchecked even though I checked it before sending it. Any ideas?
