Check Boxes

I am a fairly basic user and am writing a database where I work to store
records of visits. I have a form with five check boxes. A user should be
able to tick on any of check boxes 2, 3, or 4. If they click on check box
1, boxes 2, 3, 4, and 5 should all automatically disable and likewise if
box 5 is ticked boxes 1, 2, 3, and 4 should all disable. I can get box 1 to
automatically fill out boxes 2, 3, and 4 etc but cannot get them to be
disabled. Sorry if this is a bit confusing! Please help!

If you have five choices and the user is only allowed to make one choice you
should use an option group instead of a series of check boxes. Search help on
Option Groups.
