A friend created a form for me in access that has check boxes. However, its
not allowing to select any number of choices from the set of independent
choices. It's functioning functioning like a option button control (only 1
choice) .

When I check the format in deign which, it shows a check box. Can someone
tell me how to correct this?

It probably is an option button control. To find out for sure, click around
the check boxes at any frame. It will put a selection handle on them all. An
option group frame can be transparent, but you will see it in Design View.

I'm not clear on what I'm suppose to do or see when I click the check box at
any frame. I went to design view, clicked the check box frame and it
selected all the check boxes. Then what I'm I supposed to see or do?

If it selected all the check boxes, it is an Option Group control. You'll
need to delete it and replace it with individual check boxes. If you want to
maintain the data in a proper relational manner, you'll need a field for
each check box.
