Cant create Exchange Mailboxes when creating new user account

I am unable to create an Exchange Mailbox when creating a new user account. The user account creates fine but never creates a mailbox.
 
Hi, Are Exchange Administration Tools installed at the particular computer
you create the account from?

I might jump in now as I am sure that Chris is in bed now!

Are you using the Active Directory users and Computers MMC that is from
"Microsoft Exchange" in Start | Programs with the Advanced Features checked
( under View ) instead of from the ADUC from Start | Programs |
Administrative Tools? You will need to use that one instead now.

Also, when you installed the ESM on the DC ( via dropping in the Exchange
2000 CD and selecting the Custom Install ) did you also install the same
Exchange 2000 Service Pack as you have installed on your Exchange Server?

But are you launching ADUC from the Exchange program menu, or from the
Administrative Tools menu? You need to run it from the former, as someone
else mentioned. Are you not even prompted to create the mailbox?
