Cannot Competely Defrag in XP

In the other versions of Windows (98, 98 SE, ME, 2000), Defrag would rearrange the files that were used most often to the beginning and would make the files continuous while it was defragmenting the disk. However, with XP, not ALL files are defragged nor are they continuous. Is this normal in XP? IF NOT, how can I fix this?
