Can I write a macro to attach a form and address an email at once?

I would like to know if there is a macro that will attach a form to an email
and address the email with the "To" field at the same time. I would like to
assign this macro to a command button. So the form user can click the button
and the form will attach to an email and the email will address who it is
being sent to.
