CAN I PUT A submission form in an HTML emial

Ive been trying to put a submission form in an html email
but I can't seem to get the results to go anywhere. ie
like an email address I set it up to do so in form
properties but it doesn't work
 
The form must be accessed directly on the web server which has the FP
extensions.
