Can I map to user-defined fields when I import an address list?

I defined about 20 icon fields for additional information for the contacts in
my company. However, when I import those fields are not available to map to.
How can I make them available for the "map custom fields" portion of the
import?

by default it is not possible to map user-definied fields in Outlook:

Not in Outlook 97, not in Outlook 2007.
