Can a survey be set up through a hyperlink return email

I am sending a newsletter and want to include a hyperlink for a return email.
Is there a way that I can set the hyperlink to bring up a return email with a
simple survey that the respondent answers 6 or 7 simple questions each with
yes or no check boxes?

I am using the 2010 Beta program
 
Not without macros and even then only if the recipient is using Outlook
which can be programmed from Word (or vice versa). Unfortunately you have no
control over either.

One solution would be to create a web based form and link to that, but this
is not a task for Word but for some real web creation software.

The other more obvious solution would be to attach a protected form (in Word
97-2003 format) to an e-mail message and instruct the user to return it to
your e-mail address in the body of the message. You may then find
