Can a form have two record sources?

I have contructed a database for questionnaire purposes, which is based on
two tables and two forms. Each form is linked to the relevant table.
My problem/question is can a form be based on two record sources? (i.e. two
tables). So far we have found that when inputting data we might accidently
input the data on a wrong record. In order to avoid this we simply want to
have one form - that is based on our two tables. Is this achievable?

Use a form with a subform.
