Can a form be filled and sent in outlook?( Not attachment)

I'd like to create a form that can be filled in outlook and sent not as an
attachment but an email. Can this be done? Not using Word.

Maybe, but probably not, depending on these factors:

1) Is the recipient inside your organization?

2) Does the recipient use Outlook?

3) Does your organization use Exchange as its mail server?

4) Can you publish to the Exchange server's Organizational Forms library?

If the answer to all 4 questions is Yes, then it can be done. See http://www.outlookcode.com/d/forms.htm
