Calendar / Submitted Form

1st - I'm making a form and have three spots that a date needs to be entered.
Is it possible to have it set up so that when a user clicks on that form
field a calendar populates and they can click on whatever date needs to be
entered?

2nd - I have this form set up so that when the user fills it out and submits
it, it is sent to my email. Is it possible to make the email I recieve look
exactly like the form created in FrontPage?
