Using Outlook 98 or Outlook 2000 and exchange server 2003, all meeting
requests are received as plain email and don't end up in calendar with an
accept or decline buttons

Are these sent internally or externally? Did you force rich text format?

I'm having this problem as well. I use Win XP Pro and Outlook 2002. My
meeting requests also show up as plain email. I use IMAP at a university.
I've noticed something funny though - if I move the email to a folder other
than Inbox, I can suddenly see the accpet/decline buttons again. Weird -
it's driving me crazy. Thanks for your help!

Ps - I have all updates installed.

Any follow up on this?
