calendar csv to ical format?

how do I convert an Outlook 2000 calendar csv file to ical format so I can
import to Windows Calendar in Vista Ultimate
 
I saw a mail merge sample somewhere on the internet but can't find it now
but it's not that hard to make your own - open an ical in word and insert
the field codes in the appropriate place. This has the basics:

that word can use the CSV as the source so step 4 is not necessary.
