Calculate Field for Sick Time

I am trying to create a field on a form that will
calculate sick time at 5 hours per month, up to 40 hours
(no more)per year. The beginning of the accumulation
would always start Jan of the current year, therefore, all
sick time accumulated in the prior year needs to be zero'd
out if not used - OR - Sick time could be "rolled" over if
it is less than 40 hrs and accumulate UP TO 40 hrs. if it
gets used.

After creating this field, I need to perform a subtraction
calculation based on usage. (I already have the sick time
used totaling - so now I need to get the field to
accumulate sick time - then I can create a field to
subtract the two. HELP!
