When in a contact the business contact history box is identical to the inbox,
how do I remove this or restore it to it's original information?

Sound like the Outlook forms cache got corrupted. Delete the file or
clear it from Outlook.

Clear the Outlook forms cache, it's gotten corrupted. You may need to
run Windows Update, as the bug that usually caused cache corruption
was fixed by Outlook in 2004.
