Business Contact Manager Form won't load

When I try to open a "business contact" I get the message:

"The custom form could not be opened. Outlook will use
an outlook form instead. The form required to view this
message cannot be displayed. Contact your administrator."

After clicking OK I get another dialogue box that says:

"Array index out of bounds"

The form I need appears to be corrupted.. How can I
restore the original "business contact" form? I have
tried detect and repair and reinstalling - nothing seems
to work.

Suggestions would be greatly appreciated...
 
Clear the form cache from within Outlook:

Tools -> Options -> Other -> Advanced Options -> Custom Forms -> Manage
Forms -> Clear Cache

"OK" your way out, re-start OL and everything should be fine once again.
