Bookmarks - Keep disappearing

I am using Word XP. This has happened more than three times. My books are simply disappearing without me deleting
them. This has become a very time consuming problem as my document is about 200 pages and i cross reference
the book marks. Can some one please help me with
(1) why there are disappearing
(2) How can i get them back (without having to do them all over again - which i already did three times!)
(3) How can i prevent this from happening.
