How do I create the code requirement of someone sending me an e-mail? you
know, where you see an image then have to type it in before sending the
e-mail in order to prevent all the spam.

Can you describe this in different words? I don't undersatnd the question.
 
Just like when I signed up for this service, I was asked to type in the
letters & numbers I saw in a coded box so the receiver was sure that a person
and not a machine was sending the e-mail. How can I set this up on my
outlook? Out of 50 e-mails I might receive, only a few are genuine - the
rest is garbage from pill pushers to stock buy tipsters.

There's no way to do that in Outlook itself. However, you can subscribe to
a challenge/response service like that offered by www.spamcop.net if you
want people to have to seek your permission before they can send you mail.
