billing statement form

I am starting a small business. I am in need of a statement form. Just a
simple form that gives a total at the end.

The Northwind database has a lot of samples and there are a couple of
templates.

I'd go to eBay and buy and "old" version of QB, Pearchtree or MYOB.
 
Thank you Anton, but I do not have Xcel on my computer. I have Windows XP
and one would think there would be a simple Statement Form there.
 
Thank you Mike. I was really hoping for somthing simple from Microsoft, that
was all ready on my computer.
