Auto fill one field from a previous filed

We have a document that our office uses to send information about a patient
to insurance companys, I have pasted a paragraph below. When our users type
patient name the first time, I want it to fill in the patients name field
below. Can someone tell if this can be done and if so how I could do it(and
save it as a template for the users)
