Address field being pre-filled with <unused>

This is driving me crazy and I can't figure out what is causing it.
Very often, when I go to a page that has a form with a field named
"address" (like almost any order form), IE pre-fills it with <unused>.
This is very annoying. It's easy enough to overwrite it on a blank
form, but more than once I've gone to a page to edit my profile (for
example, to change a phone number) and IE has overwritten my existing
address with <unused> and I failed to notice this and then had a
problem with the merchant when they had no valid address for me!

Please tell me how I can prevent this annoying behavior.

Are you sure that you don't have the Google Toolbar, or some other similar
add-on, installed which is attempting to prefill the forms?
 
I do have the google toolbar, but I have the "auto fill" feature turned
off (when I go into "options" it is unchecked). Hmm. Any other ideas? I
have the yahoo and answers.com toolbars, too, but I just checked and
they don't have any auto-fill features.

And if you turn off those toolbars does the problem go away?

Check if you have any BHOs (browser helper objects) installed that might
provide the auto-fill functionality (the problem you described in not
behavior provided in IE). Although you could use Internet Options ->
Programs -> Manage Add-ons, it would probably be easier to use BHO Demon.
