**Address book location

Outlook does not use an address book file. Your address book is simply a
dynamic view of the electronic addresses contained in any Contact Folders
you have enabled as an email address book.
 
So if I back up "Outlook.pst" then all of my mailboxes, contacts & calendar
are backed up together?

Depends on what the name of your default PST file is. It's where all the
mail, calendar, contacts etc are stored.
