Add attachment to form

I have a custom form prepared, but I want to be able to put an attachment
with it. How do I do that?
 
I'm not clear on what you want to do -- attach a file to a form so that all
items created with that form have the same attachment (what would be the
point of that?) or attach a file to an item created with a custom form.
Which is it?
 
The later. I want a form for people to use, but they need to be able to
attached a document to the form and email to someone else.
 
Then, as discussed in your other thread, you must have the message control
on the form somewhere.
