Add a form

I have created a form on Word that I want to use as an email template. I
don't know how to add it to the present forms that I access from
Tools>Forms. Can anyone help me?
 
Set your mail format to rich-text. Create a new message
form. On the (Properties) page, check the box for "Use
WordMail" and provide the path to your Word .dot
template.
