Add a CONTROL SOURCE item or FIELD item

When I'm in FORMS I've added a COMBO BOX line item, but there's no option
from the FIELD or CONTROL SOURCE (?) for the item I want to show.

It's a inventory database and I've added a TABLE called BENEFICIARIES. In
this table is the Beneficiary ID (auto number) and Beneficiary Name (which I
list my family members). When I'm in FORMS I cannot choose the line item
that I added to list the Beneficiary Name, it keeps pulling only the
Beneficiary ID (number). Also, the CONTROL SOURCE only lists Beneficiary,
not BeneficiaryID like the other COMBO BOXES that were already on the form.

Help!

Did you make the combo with the wizard or by hand?

I tried both ways. I'm not sure, when I used the wizard, if I should have
added BOTH the Beneficiary ID and also the Beneficiary Name, or just one or
the other. I tried it a few different ways, but it doesn't seem to work. I
finally was able to have the field show a drop down of the Beneficiary ID
(which were just numbers 1-10). But, I want the Beneficiary Names to show in
the drop down list.
