Accounts lock after 1 bad password?

Can someone explain this convoluted workings?

I have the domain password lock out set to 5 tries. However, when
someone tries to connect to exchange or to a file share it seems that
windows hammers the account until it gets locked out... what the heck?@?!?!

If I check the security logs sure enough the account hammered away 8 or
9 times.

I've seen this happen on web servers too.. when people connect there
will be like 15 - 20 authentication requests for a single file share
mount. Can someone please explain this and how do I stop it?
 
What seems to be happening is that they will open exchange or try to
connect to a file share on a machine where they aren't directly
authenticated to.. ie ithey are logged onto domain A... but the exchange
server account is on domain B (I know.. it's not setup right.. but
we're in the midst of migrating from exchange anyway)... when I log
into exchange server (they enter their password when exchange starts) if
they enter it wrong it seems to hammer until the account gets locked
out. Why does it do this and how do I fix it?
